• 1 print of the mark if it is to be applied in black and white, or 15 prints, if in color (or 1 image by e-mail);
• Applicant's full name and address;
• Exact list of goods and services, preferably according to the 8th edition of the International Classification;
• Power of Attorney simply signed on behalf of the Applicant, with indication of the full name and position of the signatory. Neither legalization nor notarization is required. An executed Power of Attorney may be submitted within 2 months from the filing date, subject to late filing expenses;
• Priority application No. and priority date, if the Convention priority is claimed (this information may be submitted within 2 months from the filing date);
• Priority document, if any (may be submitted within 3 months from the filing date, subject to late filing expenses).
