• 1 print of the mark; if the mark is in black & white, or 12 prints, if in colour (or 1 image in electronic format);
• Applicant's full name and address;
• Exact list of goods and services, preferably according to the 8th edition of the International Classification;
• Priority application No. and priority date, if the Convention priority is claimed (this information may be submitted within 2 months from the filing date);
• Power of Attorney (a form is attached) simply signed on behalf of the applicant, with indication of the full name and position of the signatory; neither legalization nor notarization is required. This document may be submitted within 2 months from the filing date, subject to late filing expenses;
• Priority document, if any (may be submitted within 3 months from the filing date, subject to late filing expenses).
